The Registrar’s office is the gateway for a student’s admission to the school. It manages registration and student records, such as recording and upgrading of grades, issuing certificates of enrolment or completion, diploma or certificate of graduation. It also evaluates graduation requirements and issues school records or transcripts.

Parents, Students and/or school clients can access information of the Registrar’s services through the school’s website www.shs-adc.edu.ph. For inquiries of the services of the Registrar’s office you can send email messages to registrar@shs-adc.edu.ph for concerns regarding: school application, enrolment, requesting transcripts/ school records, clearance for leaving the school, updating of personal data, address and contact information and graduation requirements.

Registrar Services

School Academic Calendar

  • Provides a copy of school’s academic calendar for your guidance (visit our school’s website). Entries are subject to change as the need arises without prior notice to parents/students.

Student Application
Assists and facilitates student applications. Applications are processed starting from December to May.

  • Student applicants studying within the country (Philippines) may proceed to the Testing Office.
  • Foreign student applicants or a Filipino student-applicant who studied abroad may apply at the Registrar’s Office. Please refer to the Admission link of school’s website for the list of requirements. Only complete requirements will be processed by the Evaluating Committee within 7 working days.

Special Study Permit (foreign students)

  • Facilitates the documentary requirements for application for Special Study Permit (SSP) of officially enrolled foreign student.

School Record/Transcripts & Certificates

  • Updates the Academic Records quarterly. Assists in the issuance of school record/transcript and certificates.

Diploma or Certificate of Graduation

  • Facilitates the diploma or certificate of graduation to students.

College Application Forms

  • Facilitates in the filing out of college application forms.

Requirements for Admission for Foreign Applicants / Filipinos Studying Abroad

  1. SUBMIT the following documents for evaluation to the SCHOOL REGISTRAR
    • If the applicant is NOT A FILIPINO citizen

-Original PASSPORT
-PHOTOCOPY of passport biodata page and latest admission with valid authorised stay

    • If the applicant has DUAL CITIZENSHIP, submit a photocopy of PHILIPPINE PASSPORT and / or CERTIFICATE OF RECOGNITION as a Filipino citizen
    • Parent’s Passport (Photocopy)
    • Alien Certificate of Registration (ACR) / I-CARD (Photocopy of front and back portion)
    • BIRTH CERTIFICATE OR FAMILY REGISTRY
    • COMPLETE & ORIGINAL SCHOOL RECORDS of all Grade Levels attended and DULY SIGNED BY THE SCHOOL PRINCIPAL (from Pre-school to last grade level enrolled in); for NON-ENGLISH SPEAKING COUNTRIES, have the school records DULY AUTHENTICATED BY THE PHILIPPINE EMBASSY located in the student-applicant’s country of origin or legal residence, both in English and in his national alphabet
    • CERTIFICATE OF ENROLLMENT of the current school year or  CERTIFICATE OF COMPLETION of last completed school year
    • SPECIAL STUDY PERMIT (SSP) for a foreign student enrolled in the Philippines of the current school year or of last completed school year
    • 2 pieces of 2×2 most RECENT IDENTICAL PICTURES with white background

B. INCOMPLETE REQUIREMENTS WILL NOT BE PROCESSED.

Procedure for Online Requests of Student's Official Records

1.     For inquiries, email your questions to registrar@shs-adc.edu.ph

2.     For document requests, email your requests to  jovelyn.mirafuentes@shs-adc.edu.ph

- Registrar will reply to confirm your request.

- Registrar will ask if you prefer the documents:

·       in soft copy (will be sent to your email)

·       in hard copy ( picking up of the documents will be scheduled in gate 1)

- Registrar will advise how payment will be done online through BDO/Metrobank

3.     Send a clear copy of the validated deposit slip (scanned/pictured) to jovelyn.mirafuentes@shs-adc.edu.ph and subject of the email should be your Family Name and First Name

4.     Ms. Jovelyn will verify your payment with the Accounting office using the scanned/pictured validated deposit slip

5.     Releasing of documents according to their preferred method.
TIME FRAME of Requests:

1-2 working days: (Form 137) Transcript of Records
1-2 working days: Certificates

 

ALUMNI:

a)    TOR requests of Alumni should be requested from his/her last school attended.

b)    SHS-Ateneo de Cebu Registrar’s Office may grant to Alumni request if they submit a Letter of No Objection issued by the Registrar of his/her last school attended.

> Fresh Senior High School graduates can request a copy of their Transcript of Records until said document has been transferred to the school/college/university where he/she is officially enrolled in
